< PreviousVARIOUS PLANS Many employers offer a higher level of coverage than the basic plan, and there is a range to choose from. Policies vary from the minimum legal requirements of SHIC to comprehensive international plans. SHIC benefits cover, at minimum, up to CI$100,000 major medical per year with a CI$1 million lifetime maximum for each person. This is mostly towards hospitalisation, surgery, dialysis, chemotherapy, oncology radiation and emergency care. SHIC plans have limited local outpatient benefits and any overseas care is usually restricted to the kind of major care unavailable in Cayman. The more comprehensive plans offer wider access to overseas services, larger per annum and lifetime allowances (up to US$2 million or higher could be available in a calendar year), fuller prescription coverage, more outpatient services and options to have dental, routine optical and life insurance benefits. According to AON Cayman, most health insurance plans fall within the following categories: SHIC: Minimum plan required by law. Enhanced SHIC: Minimum plan required by law, plus higher outpatient benefit limits; may include dental and/or vision coverage. Mid-level: Higher annual maximum limits, adequate outpatient benefit, along with dental and vision coverage options. Comprehensive major medical: Lifetime maximum limit of US$2 million, with wider access to overseas care and less out-of- pocket expenses. Costs vary and comprehensive plans can be pricey. A basic SHIC plan without dental and vision costs around CI$167 per month per employee. A medical plan with enhanced outpatient benefits averages about 20 to 30% higher than SHIC. The premium rates are about double for a couple and triple for a family rate. Adding one dependent is about twice the single monthly costs and adding two or more dependents is usually in the range of three times the employee-only monthly premiums. As most employers in Cayman do not continue to offer health insurance benefits to people after retirement, many are unable to afford to retire on-island. This can save loved ones a huge amount of anxiety should you become incapacitated while unwell, with instructions protected by the Health Care Decisions Law, 2019. The legal document, referred to as an advance health care directive, outlines preferred medical treatments and procedures such as ventilator and feeding tubes, resuscitation and life-support. It also allows for appointment of a proxy to make healthcare decisions on his or her behalf, if the directive-maker wishes. The directive will only take effect once the directive-maker is deemed to be mentally incompetent by a doctor and can be revoked at any time while they are still mentally competent. Mentally incompetent means that you are unable to make a decision and do not possess the faculties to understand the nature and effect of decisions. Examples of being mentally incompetent include being sedated for medical reasons or suffering from a medical condition leaving you unable to make informed decisions (on many occasions, this might only be a temporary situation). HOW TO COMPLETE AN ADVANCE HEALTH CARE DIRECTIVE As well as being available for collection at Cayman Islands Hospital, Faith Hospital, and district health centres, the ADVANCE HEALTHCARE DIRECTIVES Cayman Health 2021 HEALTH ADMINISTRATION AND POLICY 50advance health care directive form is available from the Ministry of Health website ministryofhealth.gov.ky/sites/ default/files/health-care-directive- and-proxy-appointment-form.pdf. According to the 2019 law, a person must be older than 18 and mentally competent to file an advance directive. The form must be signed in the presence of a doctor, who will witness and sign it, as well as one other witness who is not the directive-maker’s beneficiary or proxy. The full law can be accessed through the following link: ministryofhealth.gov.ky/sites/ default/files/Health%20Care%20 Decisions%20Law%2C%202019. PDF. If you have filed an advance health care directive, give a copy to your lawyer and doctor, let your family know it exists, and add it to the Medic Alert Pack on your fridge, if you have one. These packs are available from Hazard Management Cayman Islands and provide first responders with quick and ready access to information about your medical history, medications and allergies. You can also take your directive to the HSA records department and ask for it to be attached to your records. Individuals are encouraged to contact the Ministry of Health on 244-2318 or 244-2377 or visit the Ministry’s website (ministryofhealth.gov.ky) if they have any questions.
